Address Change Request Form

Address changes for non-active students must submit the Address Change Request Form.

Please note: If you are an attending JIU student, you must update your address through the student portal, which may be accessed at https://my.jiu.edu. This form will not be accepted for active students.

Certificate Request Form

In order to process your request please submit the Certificate Request Form. Please note that requests for certificates can be submitted at any time, but will not be processed until you have met all graduation and financial requirements. Once these requirements have been met, your certificate will be sent to the address listed on the form. Forms will remain on file for 30 days after receipt; if you have not fulfilled all graduation and financial requirements within the 30 day window, your request will be denied. There is no charge for requesting a certificate.

Diploma Request Form

In order to process your request, please complete and submit the Diploma Request Form along with payment. Please note that requests for diplomas can be submitted at any time, but will not be processed until you have met all graduation and financial requirements.  Once these requirements have been met, two copies of your diploma will be sent to the address listed on the form. Forms will remain on file for 30 days after receipt; if you have not fulfilled all graduation and financial requirements within the 30 day window, your request will be denied and your payment will be returned to you.

The cost of requesting a diploma is $100 for Undergraduate and Masters level students and $350 for Doctorate Students.

Paying by Check

If paying by check, please print and mail the Diploma Request Form along with payment to:

For an additional copy of a diploma, a Second Diploma Request Form, along with a fee of US $50 can be submitted electronically or mailed to JIU. The student can enter credit card information on the form or mail a check.

Enrollment, Degree, or Certificate Verification Request

Students in need of an official letter stating their enrollment status, degree conferral date, or certificate completion date will need to submit the Enrollment Verification Request Form. JIU offers the following types of enrollment verifications:

  1. Enrollment Status Verification – Detailed enrollment verification letters will be used to verify your enrollment status as a current student at JIU. The following information will be included on a detailed enrollment verification letter:
    • Student name and Student ID number
    • Current enrollment status and term dates
      • NOTE: Please choose “All Terms Attended” if you require your complete enrollment status history included on the letter.
    • Program name
    • Expected graduation date
  2. Degree Conferral Date Verification - Graduation date verification letters will be used to verify a student’s conferred degree with JIU. The following information will be included on a graduation date verification letter:
    • Student name and Student ID number
    • Conferred graduation date
    • Awarded degree type
  3. Certificate Completion Date Verification
    • Student name and Student ID number
    • Completion date
    • Certificate type
  4. Non-Enrollment Date Verification – Non-enrollment date verification letters will be used to verify a student’s inactive status with JIU. The following information will be included on a non-enrollment date verification letter. No third party requests- ONLY student and institution requests will be accepted. 
    • Student name and Student ID number
    • Program name
    • Inactive status date
    • Reason for inactive status
  5. Withdrawal Date Verification – Withdrawal date verification letters will be used to verify a student’s withdrawn status with JIU. The following information will be included on a withdrawal date verification letter:
    • Student name and Student ID number
    • Program name
    • Withdrawal date

In-School Deferment Form Certification

Students are responsible for contacting his/her lender(s) to obtain the necessary form. Each lender has a form, which often varies with the information required and a format based on their processing needs. Please complete the student section(s) on the lender's form. This typically includes personal information including the student name, address, account number or other identifier. Then submit the form to the JIU Office of the Registrar via fax at 303.784.8547 or by mailing the form to:

Name Change Request Form

JIU students that are requesting a name change on their academic record will need to submit the Name Change Request Form and attach appropriate documentation (appropriate documentation is listed on the form).  Please note: Name Change Request Forms will be rejected if appropriate documentation is not submitted with the request.

State Licensure Exam Submission Form

Students must submit the State Licensure Exam Submission Form with attached exam results that correlate with their state required licensure exam. This form and exam results must be submitted prior to registration for student teaching courses.

Transcripts

JIU has the right to withhold official documentation (e.g. official academic transcripts, enrollment verification and diplomas) if the student has not made arrangements for payment for current tuition and fees, or if the student has an outstanding balance on his/her account.  

  • Students with an outstanding balance may request one (1) unofficial academic transcript that will be mailed directly to the student and not directly to any third party.  
  • Students with no outstanding balance may request unofficial transcripts at any time using the Transcript Request Form.  

Official JIU Transcripts

In order to process your request, please complete and submit the JIU Transcript Request Form along with payment. Please note that official JIU transcripts are $15 each, payable by check or credit card.

Please allow 3-7 business days for processing once the request is submitted.

Unofficial JIU Transcripts

Unofficial transcripts are free of charge and available online to current JIU students. Please use the following steps to access your unofficial transcript online:

  1. Click Student Login and enter your username and password to enter the student portal
  2. From your student portal, select Academics (on the left)
  3. Within the Academics menu, select Degree Audit
  4. Click the Unofficial Transcript link

If you need an unofficial transcript to be sent directly from the Office of the Registrar, please complete the JIU Unofficial Transcript Request Form using EchoSign.
